Making Primary Care Data Reports Meaningful to the Public
Between August 2022 and March 2023, PERC partnered with the Greater Hamilton Health Network (GHHN), one of 54 Ontario Health Teams, to create lay-friendly visualizations of local/provincial primary care data.
PERC researchers facilitated participatory design sessions with:
13 patient/caregiver partners from PERC and GHHN
3 GHHN staff representatives
2 PERC-TUTOR Primary Care Trainees
We also engaged GHHN primary care providers, communications staff and a graphic designer to iteratively refine prototypes. Our partners identified: the target audience, scope of work, data to highlight, design features, key messages, and venues to share this work.

Primary care is the first point of contact between a patient and the health care system. Our first visual gives an understanding of what primary care is, who primary care providers are, and the impact of having a regular primary care provider on health outcomes.
